Clinical Pharmacotherapy Specialist Adult Transplant Position

As a Clinical Pharmacotherapy Specialist Adult Transplant, you will be part of a team that comprises more than 200 locations throughout the New York area, including five inpatient locations, a children's hospital, three emergency rooms and a level 1 trauma center.

We are seeking a highly motivated and experienced Clinical Pharmacotherapy Specialist Adult Transplant to join our team.
The successful candidate will be responsible for developing and maintaining a high-level practice in the area of Adult Transplant Services.
Works under minimal supervision to troubleshoot challenges related to the delivery of patient care, clinical program management, and staff supervision.
Responsibilities include:
- Developing, implementing, and sustaining clinical pharmacotherapy programs.
- Providing expertise for Medication Use Evaluations.
- Precepting Pharmacy Residents, mentoring Pharmacy students, and engaging in Scholarly activity.
- Participating in Hospital committees, as identified and assigned by the Director of Clinical Pharmacy Services.
- Documenting and reporting clinical interventions, adverse drug reactions, and medication errors.
- Developing and implementing clinical practice guidelines to enhance pharmaceutical care and medication safety.
- Evaluating and reporting measures of patient encounter productivity and identifying methods to enhance efficiency of such encounters.
- Developing and maintaining policies and procedures that optimize pharmacy operations while meeting accreditation standards and applicable regulations.
- Contributing to the professional development of staff, including Pharmacists and Clinical Pharmacists.
- Providing oversight for quality assurance of clinical pharmacy programs to improve quality of care.
Requirements include a Doctor of Pharmacy degree from an ACPE accredited school of pharmacy, New York State Pharmacist Licensure, PGYI pharmacy residency training, and PGY2 pharmacy residency training or equivalent salaried professorship at a school of pharmacy. Board certification in Pharmacotherapy or other specialty is required within 18 months of hire. Self-motivation, good communication, organizational, and interpersonal skills are essential.
