Responsibilities:

  • Provide high-quality, patient-centered care to individuals in their homes, focusing on their unique needs and goals.
  • Conduct thorough assessments, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless care transitions.
  • Collaborate with patients, families, and caregivers to educate them on disease management, medication adherence, and self-care techniques.
  • Monitor patient progress, adjust care plans as needed, and communicate effectively with healthcare teams to ensure optimal outcomes.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.

Requirements:

  • Current Ohio RN License
  • Minimum of 1 year of recent home health nursing experience (private duty does not qualify)
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience
  • American Heart Association BLS
  • Valid U.S. Driver's License
