  • Resource Development: Develop and maintain a network of community resources to support clients' needs, including transportation, childcare, and other essential services.
  • Communication and Collaboration: Maintain effective communication with clients, family members, and healthcare providers to ensure timely and accurate information exchange.
  • Documentation and Reporting: Maintain accurate and timely documentation of client interactions, care plans, and service coordination activities.

Requirements:

  • Education: Bachelor's degree in a human services or related field required; Master's degree preferred.
  • Experience: Minimum of two years of experience providing direct services to seriously mentally ill patients/clients required.
  • Skills: Effective oral/written/interpersonal communication skills required; bilingual skills preferred.
