Job Title: Wellness Care Manager

Job Summary:

We are seeking a highly skilled and compassionate Wellness Care Manager to join our team at VNS Health. As a key member of our care management team, you will work closely with clients, families, and healthcare providers to develop and implement personalized wellness plans that promote optimal health and well-being.

Responsibilities:

  • Conduct comprehensive assessments to identify client needs and develop tailored wellness plans
  • Coordinate and integrate services with healthcare providers, community resources, and family members to ensure seamless care delivery
  • Monitor client progress, track outcomes, and make adjustments to wellness plans as needed
  • Provide education and support to clients and families on health-related topics, including disease management, medication adherence, and healthy lifestyle choices
  • Collaborate with interdisciplinary teams to ensure comprehensive care and address complex client needs
  • Document client interactions, progress, and outcomes in a timely and accurate manner

Requirements:

  • Bachelor's degree in a human services or related field required; Master's degree preferred
  • Minimum of two years of experience providing direct services to seriously mentally ill patients/clients
  • Effective oral and written communication skills, with ability to work with diverse populations
  • Basic computer skills and proficiency in electronic health records
  • Bilingual skills a plus
