1) Completion of AHCA approved targeted case management training within 3 months of employment.

2) Certification through the Florida Certification Board (FCB) is required within 6 months of employment.

Must be at least 21 years old.

Negative Drug Screening is mandatory prior to employment and throughout the duration of employment.

Must pass a clear Criminal Background Check during employment with Suncoast Center, including local checks prior to employment and annually, along with DCF Level II checks as required. A valid Florida Driver's License is necessary, with motor vehicle checks conducted prior to employment and annually, maintaining an acceptable driving record throughout employment. A reliable vehicle, valid registration, and valid vehicle insurance are required, in compliance with Florida Laws and Administrative Codes, including fingerprinting and verification of good moral character.

Working Conditions: A valid Florida driver's license and an acceptable driving record are required. Must meet minimum personal automobile liability insurance levels. The role involves both indoor and outdoor work, with outreach to various locations. Exposure to extreme heat and humidity may occur during summer months. The schedule may vary.

Proficiency in computer usage is essential.

Job Responsibilities:
  • Assist individuals deemed incompetent to stand trial or found not guilty by reason of insanity.
  • Guide clients in understanding the legal system to facilitate evaluations for competency. Provide individual and group training aimed at achieving competency, including psycho-educational and stress/time management training, while reviewing the conditions of the conditional release order. Support clients in their efforts to remain in the community and provide additional case management services as required.
  • Serve as a liaison for clients at a Florida State Hospital by attending treatment team meetings, either in person or via telephone, visiting quarterly, and maintaining necessary documentation in accordance with case management regulations.
  • Conduct assessments and document clients' symptoms, diagnoses, and interventions, ensuring all program criteria are met for case initiation.
  • Complete all necessary documentation to initiate the client into Targeted Case Management (TCM) when appropriate.
  • Develop and maintain an accurate assessment and service plan within 30 days of case initiation, with updates every six months thereafter.
  • Establish prioritized, behaviorally descriptive, and realistic desired changes with clients, creating a feasible plan for their achievement with appropriate timelines.
  • Ensure Continuity of Care in compliance with Florida Statutes and Administrative Codes, including Case Management Regional Operating Policy and Florida Medicaid Manual.
  • Advocate for clients at relevant staff meetings, court hearings, etc., or arrange for a substitute advocate.
  • Connect clients with necessary resources to address their needs and monitor the effectiveness of these interventions.
  • Utilize appropriate crisis intervention skills and proactively prevent crises where possible.
  • Stay informed about psychotropic medications, their side effects, and DSM IV diagnoses, recognizing when medical services are warranted.
  • Conduct a minimum of one face-to-face contact monthly and one bi-monthly home visit with all clients in TCM, documenting reasons for any failures to meet these requirements. Meet other clients at least bi-monthly.
  • Meet productivity standards consistently.
  • Effectively utilize support funds to meet client needs.
  • Complete all progress notes and documentation (e.g., timecards, outcome measures, FARS, etc.).
  • Prepare all required judicial reports every six months or as requested by the courts.
  • Monitor conditional releases for appropriate clients and advocate for the least restrictive community placement following incarceration.
  • Attend all court dates as requested by the Public Defender's Office.
