Job Summary

We are seeking a highly skilled and detail-oriented MDS Coordinator to join our team at Gallatin County, MT. As a key member of our healthcare team, you will be responsible for coordinating and conducting periodic resident assessments, developing and implementing care plans, and ensuring compliance with established guidelines and regulations.

Key Responsibilities
  • Resident Assessment and Care Planning: Coordinate and conduct periodic resident assessments, develop and implement care plans, and ensure compliance with established guidelines and regulations.
  • Data Collection and Reporting: Collect and compile data for resident assessments, complete MDS assessments, and report results to relevant stakeholders.
  • Audits and Assessments: Conduct regular audits, including pressure injury audits, AIMS and behavior quarterly assessments, and pharmacy reviews.
  • Care Conference Coordination: Coordinate care conferences, including sending out notices to staff, reviewing EHR for medication questions, and completing care conference notes.
  • Scheduling and Records Management: Review medical records, enter admission orders, and assess admission orders for possible contraindications, omissions, or irregularities.
Requirements
  • Education: Bachelor's degree in nursing from a National League of Nursing (NLN) accredited school of nursing.
  • Experience: Three (3) years of related work experience and/or a RAC-CT (Resident Assessment Coordinator - Certified) certification.
  • Licenses and Certifications: Valid driver's license issued by the state of Montana, and ability to provide own transportation for out-of-office job duties.
