POSITION SUMMARY

This role is situated within the Gallatin Rest Home and entails supporting the MDS Team in conducting resident evaluations, interviews, and care planning.

The position is accountable to the Rest Home Administrator and does not oversee any staff.

Assist in the preparation of the Minimum Data Set (MDS) documentation for nursing home resident evaluations.
Examine individual resident records for nursing, therapy, physician, and interdisciplinary team (IDT) documentation and evaluations. Perform resident interviews and assessments (e.g., vision, hearing, etc.). Conduct staff interviews to gather and compile necessary information for assessments.

Facilitate the creation of the relevant MDS assessment report for each resident, integrating all information gathered during interviews, chart reviews, and other data collection efforts, while also interpreting guidelines in the MDS manual.


Conduct audits including but not limited to:

Pressure Injury Audit – a weekly review that involves assessing and documenting any pressure injuries, updating reports, entering data in EHR, notifying families, and occasionally accompanying the Wound Clinic on rounds.

AIMS and Behavior Quarterly Assessments – scheduled for January, April, July, and October.
Assist in updating resident care area narrative assessments and care plans to ensure adherence to established timelines and standards.

Review records and MDS assessments, conduct staff and resident interviews, and develop initial care plans including problem statements, objectives, and interventions.

Disseminate preliminary care plans to other care team members and nursing staff for feedback and approval.
Review medical records for approved admissions and compile relevant documents to be included in resident/patient files.
Enter admission orders for new residents/patients.

Evaluate admission orders for potential contraindications, omissions, and irregularities, and report findings to staff for follow-up.
