Residential Program Coordinator

Support, Inc. is seeking a highly motivated and dedicated individual to join our team as a Residential Program Coordinator. This role is part of an interdisciplinary team that includes nurses, behavior therapists, and public benefits administrators.
Key Responsibilities:
- Train community providers and staff, develop care plans, and resolve problems.
- Manage a caseload of consumers with intellectual and developmental disabilities who live in the community in contract community homes, apartments, or their own family homes.
- Oversee all aspects of the consumer's care and ensure each experiences a high quality of life based on their abilities.
- Carry out monitoring and quality assurance responsibilities in accordance with the organization's policies and procedures, personnel manual, and applicable laws and regulations.
Requirements:
- Associate's Degree or equivalent.
- At least 2 years of related experience and/or training.
Preferred Qualifications:
- Bachelor's Degree or equivalent.
- 2 to 5 years related experience and/or training.
- An equivalent combination of education and experience may be considered.
Work Environment:
The work environment is varied between remote work, office, and community locations, including host homes and hospitals. This position requires a personal vehicle for transportation to effectively monitor and coordinate care.
