Job Title: Direct Support Specialist

Frontier Health is seeking a dedicated and compassionate Direct Support Specialist to join our team. As a Direct Support Specialist, you will play a vital role in supporting individuals with Intellectual and Developmental Disabilities in achieving their highest level of independence.

Job Summary:

The Direct Support Specialist will provide education, instruction, and support to individuals with Intellectual and Developmental Disabilities, enabling them to make informed choices and exercise their individual preferences. This role requires flexibility, strong communication skills, and the ability to work effectively in a team environment.

Responsibilities:
  • Support individuals in achieving their goals and outcomes, as outlined in their Individual Support Plan (ISP).
  • Provide transportation services, adhering to Frontier Health's Transportation policies and maintaining a safe driving record.
  • Implement and document outcomes of individuals in ISPs, ensuring accurate and thorough documentation.
  • Assist individuals with daily living skills, including personal hygiene, meal preparation, and laundry duties, as identified in the ISP.
  • Support individuals in making informed decisions regarding their employment and community involvement.
  • Facilitate meaningful discussions about employment and assist individuals in exploring their understanding of work, motivation to work, likes and dislikes regarding work.
  • Provide on-the-job training and support to individuals in the community to maximize their potential for success in community employment.
  • Monitor individuals' work to ensure tasks/assignments are completed correctly and provide prompts/reminders as needed.
  • Attend and participate in regularly scheduled staff meetings and inservices, as well as individual program planning staffings, as needed.
  • Communicate pertinent information with supervisors in a timely manner and maintain positive and professional working relationships with others involved in a person's service delivery.
Requirements:
  • High School Diploma/GED preferred with applicable experience.
  • Valid Tennessee driver's license Class D, F Endorsement, and eligibility for FH drivers' insurance.
  • Experience in intellectual, developmental, and/or mental health disabilities preferred, according to the populations served.
  • Verbal/written communication skills, ability to maintain composure in difficult situations, and ability to safely operate a 15-passenger van or other vehicle as assigned.
Working Conditions:

This role requires working in a fast-paced environment, with flexibility to adapt to changing situations and priorities. The Direct Support Specialist will be required to work in various settings, including residential, community services, and employment services.
