Key Responsibilities- Assume responsibility for the delivery of pharmaceutical care to the specific patient care area.
- Coordinate the use of human, material, and informational resources to ensure timely, efficient, and cost-effective care.
- Review patient medication profiles to guard against drug sensitivities, contraindications, duplications, and major drug interactions.
- Provide in-depth drug information and other informational materials to healthcare professionals.
- Make rounds with medical services and provide patient discharge medication consultations.
- Doctoral Degree in Pharmacy (PharmD) or Bachelor's Degree in Pharmacy (BSPharm) with a minimum of 2 years of experience.
- Licensed Pharmacist in the state of Pennsylvania.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
