Key Responsibilities:

• Conduct thorough assessments to evaluate patients' mobility, coordination, strength, and current level of function.

• Develop and implement individualized plans of care to address patients' occupational therapy needs, goals, and outcomes.

• Deliver occupational therapy services in accordance with prescribed physician orders and agency policies.

• Maintain accurate and timely documentation of patient progress, modifying plans as needed, and determining discharge care plans as appropriate.

• Collaborate with healthcare team members, physicians, and family members to ensure seamless care coordination and effective communication.

• Supervise and mentor other personnel as applicable.

Requirements:

• Master's degree in Occupational Therapy from an accredited university/program.

• Current, unrestricted state license as an Occupational Therapist in the state of practice.

• Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.

Preferred Qualifications:

• Strong organizational and communication skills.

• Attention to detail and time management skills.

• Proven relationship-builder with experience in home health therapy and completion of OASIS documentation.
