Key Responsibilities:
- Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
- Utilize company and community-based resources to establish a safe and effective case management plan for members.
- Collaborate with patients, families, and healthcare providers to identify and initiate referrals for social service programs.
- Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
- Partner with the care team to advocate for patients and families, ensuring their needs and choices are fully represented and supported.
Requirements:
- 4 years of case management, community outreach work, or volunteering experience.
- 2 years of experience working or volunteering within the Community Health setting in a field-based healthcare role.
- 2 years of experience working or volunteering within the local Community, with demonstrated knowledge of culture and values within the Community and familiarity with the resources.
- Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications.
- Access to a designated quiet workspace in your home, with the ability to secure Protected Health Information.
Preferred Qualifications:
- Bachelor's Degree (or higher) in a health-related field.
- Community Health Worker (CHW) accreditation.
- Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant experience.
- Care management experience.
- Knowledge of Medicaid/Medicare population.
- Knowledge and/or experience with behavioral health or substance use disorders.
