EPIC Rehabilitation and Nursing at White Plains is a MODERN *5 Star* 160-bed skilled nursing and rehabilitation facility that commenced operations in 2020. We invite you to become part of a network of facilities across New York State. Our mission centers around "Rehabilitation & Healthcare", focusing on the rehabilitation of patients to reintegrate them into their daily lives. For residents who require long-term care, we strive to enhance their living conditions while maximizing their quality of life.

  • The Rehabilitation Therapy Assistant is tasked with organizing and executing physical therapy programs that are planned and supervised by the Physical Therapist, aimed at aiding residents who have experienced functional decline.
  • This role involves assisting in the planning, organizing, developing, and delivering of Physical Therapy services under the guidance of the Physical Therapist, ensuring the highest standards of resident care are upheld.
  • Implement treatment plans for designated residents.
  • Accurately enter billing information in a timely manner.
  • Report on resident progress during meetings, such as UR Meetings and Patient/Family Conferences, as necessary.
  • Collaborate with other rehabilitation team members to synchronize physical therapy with additional therapeutic activities.
  • Provide residents with appropriate equipment as needed.
  • Document therapy sessions in a clear and detailed manner that reflects the treatment provided and the resident's response.
  • Complete routine documentation as required, adhering to established charting and documentation policies. Ensure all entries in residents' medical records are signed and dated.
  • Work cooperatively with all departments and staff to foster a harmonious working environment that promotes optimal resident care.

Qualifications:

  • Graduate from an accredited Physical Therapy Assistant program.
  • Licensed in New York State as a Physical Therapist Assistant, with a valid license in good standing.
  • Previous experience in a skilled nursing facility (SNF) is highly preferred.
  • Ability to perform well under pressure and collaborate effectively with all staff members.
  • A genuine desire to work with elderly, chronically ill, and disabled individuals.
  • Exceptional customer service skills.
  • Strong verbal communication skills with residents, families, and staff.
  • Treat residents and colleagues with respect, dignity, and compassion.
  • COVID-19 vaccinations are encouraged but NOT REQUIRED; we continue to provide COVID-19 vaccinations based on CDC eligibility guidelines.
