Job Title: Social Worker

Job Summary:

The Brookdale Hospital Medical Center is seeking a skilled Social Worker to join our team. As a Social Worker, you will play a vital role in providing comprehensive social work services to patients and their families.

Responsibilities:

  • Evaluate patients' social, psychological, environmental, and financial factors to determine their needs and develop a plan to address them.
  • Provide counseling and supportive services to patients and their families to help them cope with chronic illness, lifestyle changes, and treatment options.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure timely referrals to community agencies and resources.
  • Represent patients' interests and advocate for their needs within the healthcare system.
  • Monitor and supervise transportation referrals to ensure timely and appropriate services.
  • Communicate regularly with financial offices to provide information exchange and enhance Accounts Receivables services.
  • Assist staff in reporting cases of patient abuse, neglect, or domestic violence.

Qualifications:

  • Masters Degree from a program accredited by the Council on Social Work Education.
  • Complete a course study with specialization in clinical practice.
  • With degree, no experience required but internship or some experience with treatment of patients in hospital or other medical facility preferred.

Knowledge and Skills:

  • Maintain current knowledge of treatment options, quality indicators, infection control, patient safety issues, patient rights, medical legal issues, reimbursement rules, regulations, billing, and receivables.
  • Skills in oral and written communication and interpersonal skills to interact, develop, and sustain sound working relationships with patients, families, multidisciplinary professionals, other Hospital departments, and outside agencies.
  • Ability to fully utilize BHMC's electronic record systems, equipment, and other healthcare and billing systems relevant to this position.

Licenses, Certifications:

  • New York State Social Worker License.

Physical Requirements:

  • Position requires prolonged periods of standing, reaching, walking throughout the working day.
  • Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
