About the Role

We are seeking a highly skilled and compassionate Social Worker to join our team at The Jewish Board of Children and Family Services. As a Social Worker, you will play a critical role in supporting adults with serious and persistent mental illness to live in the community with as much independence as possible.

Key Responsibilities
  • Evaluate case data and perform treatment processes to analyze and evaluate course results
  • Obtain, analyze, and evaluate case data to inform treatment decisions
  • Record material and case summaries to maintain accurate documentation
  • Conduct assessments, unit inspections, and complete clinical documentation in a timely manner
  • Provide social work interventions based on assessment of resident's need and case service plan
  • Document all resident contact in progress notes
  • Conduct services with residents using evidence-based practices specifically Critical Time Intervention
  • Complete all intake assessments and coordinate intake process for new residents
  • Prepare integrative Person-Centered Service plan based on assessments from referral source and in collaboration with resident
Requirements
  • Master's Degree in Social Work (MSW) and LMSW obtained within 6 months of hire
  • Previous work with individuals with severe and persistent mental illness and substance abuse a plus
