About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at YesCare. As a LPN, you will play a vital role in providing high-quality patient care in a correctional setting.

Responsibilities
  • Contribute to the assessment of patients' health status and develop individualized care plans.
  • Participate in the development and modification of patients' plans of care, ensuring effective and safe nursing practices.
  • Perform delegated tasks, activities, and functions consistent with prior educational preparation and within the scope of practice.
  • Provide direct basic care to assigned patients, ensuring their physical and emotional needs are met.
  • Implement patient education based on established teaching plans, promoting health literacy and self-care.
  • Utilize a problem-solving approach to decision-making, ensuring patient-centered care.
  • Monitor scheduling and consult processes, ensuring seamless coordination of care.
  • Participate in Quality Improvement initiatives, contributing to the enhancement of patient care and services.
  • Collaborate with Regional Medical Director, site medical providers, and other site nursing leaders to ensure comprehensive patient care.
