Ambulatory Care Pharmacist Position Overview

Northwest Medical Homes, LLC is a premier primary care facility dedicated to delivering outstanding patient care while nurturing a supportive environment for our team members. Key Responsibilities:

  • Deliver evidence-based pharmacotherapy recommendations and patient education.
  • Identify, prevent, and resolve medication therapy issues by analyzing patient medication regimens, assessing the use of first-line medications, monitoring dosages, suggesting therapy or dosage adjustments when necessary, and evaluating formulary and cost implications for patients. Provide counseling on medication regimens.
  • Order, interpret, and monitor laboratory tests.
  • Manage adverse drug reactions and prevent drug-induced or drug-exacerbated conditions.
  • Develop and oversee medication care plans, communicating these plans to patients and the interdisciplinary team.
  • Document identified medication therapy issues and clinical outcomes.
  • Consult with clinic physicians, other healthcare providers, and team members to support individualized patient care.

Retrospective Medication Utilization Review:

  • Collaborate with third-party payers or government entities to reduce care costs and enhance patient outcomes.
  • Evaluate medication usage patterns for potential medication-related issues or optimization opportunities, ensuring compliance with medication fills, assessing therapeutic duplications from multiple providers or insurance companies, and making recommendations based on findings.

Pharmacy Services Billing:

  • Optimize patient medication and provide education through Annual Wellness Visits, Chronic Care Management, Co-Consults (Incident-to billing), and Transitions of Care.
  • Utilize Outcomes MTM to conduct Comprehensive Medication Reviews (CMR) and follow up on Targeted Intervention Programs (TIPs).
  • Document the number of office visits, consultations, and clinical outcomes of recommendations.

Quality Improvement Leadership:

  • Lead and direct quality improvement initiatives that enhance patient care and operational efficiency.
  • Analyze quality improvement metrics and develop strategies to improve outcomes for Medicaid, Medicare, and Commercial plans.
  • Collect, record, and interpret data to implement strategies in collaboration with clinic or company administrative and clinical leaders.
  • Track and present results of improvement efforts and ongoing clinical process measures to management.

Qualifications: A graduate of an accredited College of Pharmacy with a Doctor of Pharmacy (PharmD) degree. Completion of an accredited PGY1 Residency is required, with a PGY2 Ambulatory Care Residency preferred. Must possess a license or be eligible to practice pharmacy in the State of Oregon. Board Certification is required within one year of hire, with BCACP preferred. Demonstrated expertise in ambulatory care pharmacy practice is preferred, with experience in assessing patient medications, educating patients, documenting care plans, and consultations in an electronic medical record. Ability to precept pharmacy students and residents is a plus. Strong communication skills, collaborative work ethic, effective time management, problem-solving abilities, and the capacity to work independently with minimal supervision are essential.


Work Schedule:

Four 8-hour days dedicated to direct patient care, with one day per week allocated for administrative tasks from home.


Compensation:

Annual salary of $145,000 ($87/hour) plus potential bonuses.
