- Manage Adverse Reactions and Drug-Induced Conditions: Recognize and mitigate potential adverse reactions, ensuring patient safety and well-being.
- Develop and Monitor Medication Care Plans: Collaborate with patients and healthcare teams to create and implement personalized medication plans.
- Provide Consultation and Education: Offer expert guidance and education to patients, healthcare providers, and other team members to promote optimal patient care.
- Retrospective Medication Utilization Review: Conduct thorough reviews of patient medication use patterns to identify areas for improvement and optimize medication therapy.
- Billing and Reimbursement: Ensure accurate and timely billing for pharmacy services, including patient medication optimization and education.
- Quality Improvement: Lead and direct quality improvement initiatives to enhance patient care and operational efficiency.
- Doctor of Pharmacy (PharmD) Degree: Graduation from an accredited College of Pharmacy.
- PGY1 Residency Completion: Completion of an accredited PGY1 Residency program.
- PGY2 Ambulatory Care Residency (Preferred): Completion of a PGY2 Ambulatory Care Residency program.
- State Licensure: Eligibility to practice pharmacy in the State of Oregon.
- Board Certification: Board Certification within 1 year of hire, with BCACP certification preferred.
- Competitive Salary: $145,000 annually, with bonus potential.
- Flexible Schedule: Four 8-hour days of direct patient care, with one day per week dedicated to administration and professional development.
