Job Title: Physical Therapist

Ambassador Resources is seeking a skilled Physical Therapist to join our team in Passaic County, NJ. As a Physical Therapist, you will have the opportunity to work with a growing In-Home Therapy Services practice, providing outpatient PT rehabilitation services in an in-home setting for the older adult population.

Job Summary:

This position is responsible for evaluating and treating patients in their homes, assessing their needs, and developing individualized therapy programs to facilitate rehabilitation. You will work closely with the Director of Therapy Operations to ensure proper execution and maintenance of the Wellness Programs.

Key Responsibilities:
  • Evaluate and treat patients in their homes, assessing their needs and developing individualized therapy programs
  • Work closely with the Director of Therapy Operations to ensure proper execution and maintenance of the Wellness Programs
  • Provide thorough HEP education to patients and caregivers, planning for discharge when medical necessity no longer indicates skilled therapeutic care
  • Makes clinically necessary referrals to other disciplines within care partners as well as other agencies to address the most complex and comprehensive needs of the patient
  • Consults with other members of the Health & Wellness team to select the most appropriate therapeutic regimen consistent with the objective findings, functional needs, and capabilities of each patient
  • Adapts program and/or plan of care according to the needs of individual patients
  • Orders supplies and equipment (DME) as necessary
  • Maintains, identifies, and reports the need for repair of equipment as necessary
  • Ensures high-quality patient care, providing a caring and compassionate atmosphere that recognizes the individuals' needs and rights
  • Participates in departmental initiatives and demonstrates effective communication with all team members
  • Facilitates professional growth, development, and overall improvement of departmental performance
  • Performs other professional duties associated with the care of the patient, development of discipline-specific therapy services, and design of program delivery
Services Provided:
  • Therapeutic exercise, including aerobic capacity conditioning, balance coordination, agility training, body mechanics, and postural stabilization
  • Functional training
  • Manual therapy
  • Airway clearance techniques
  • Integumentary repair and protection
  • Use of electrotherapeutic, physical agent, and mechanical modalities
  • Falls Assessment and Interventions to reduce fall risk
  • Case Management in the home and community
Requirements:
  • Valid NJ State Physical Therapy License/Registration REQUIRED to start
  • Geriatric experience preferred
  • Interest and/or experience in lymphedema is a plus
