Job Title: Physical Therapist

Ambassador Resources is seeking a skilled Physical Therapist to join our team in Passaic County, NJ.

About the Role:

This is a unique opportunity to work as a clinical architect, helping to build the healthcare reality of tomorrow. As a Physical Therapist, you will provide outpatient PT rehabilitation services in an in-home setting for the older adult population.

Key Responsibilities:
  • Evaluate and treat patients in their homes, assessing their needs and developing individualized therapy programs.
  • Implement therapeutic techniques, including therapeutic exercise, manual therapy, and electrotherapeutic modalities.
  • Work closely with the Director of Therapy Operations to ensure proper execution and maintenance of the Wellness Programs.
  • Screen, examine, and evaluate patients, including history and systems review.
  • Provide thorough HEP education to patients and caregivers, planning for discharge when medical necessity no longer indicates skilled therapeutic care.
  • Makes clinically necessary referrals to other disciplines within care partners as well as other agencies to address the most complex and comprehensive needs of the patient.
  • Consults with other members of the Health & Wellness team to select the most appropriate therapeutic regimen consistent with the objective findings, functional needs, and capabilities of each patient.
  • Adapts program and/or plan of care according to the needs of individual patients.
  • Orders supplies and equipment (DME) as necessary.
  • Maintains, identifies, and reports the need for repair of equipment as necessary.
Requirements:
  • Valid NJ State Physical Therapy License/Registration REQUIRED to start.
  • Geriatric experience preferred.
  • Interest and/or experience in lymphedema is a plus.
