Job Summary:

We are seeking a skilled and compassionate Patient Care Coordinator to join our team at Carle Health. As a Patient Care Coordinator, you will play a vital role in providing exceptional patient care and support to our patients and families.

Key Responsibilities:

  • Assist Registered Nurses and Licensed Practical Nurses in the provision of patient care by performing assigned patient care and unit maintenance activities.
  • Provide hygiene needs of assigned patients, including AM/PM care, oral hygiene, and perineal care.
  • Provide nutritional needs of assigned patients, including tray distribution, setting up trays, and feeding.
  • Provide elimination needs of assigned patients, including Foley care, bowel movements, and bladder training.
  • Provide skin care of assigned patients, including turning, positioning, and recognizing and preventing skin breakdown.
  • Provide comfort measures, including proper positioning, back rubs, and oral care.
  • Perform measurements of assigned patients, including I&O, hemoccult, and TPR.
  • Perform additional skills as directed and supervised by the nurse, including intermittent urinary catheterization and clean dressing changes.
  • Assist with admission, transfer, and discharge of patients.
  • Complete assigned aspects of admission process, including vital signs, height and weight, and room setup.
  • Set up equipment for care, including IV poles and admission kits.
  • Store and transfer belongings, including dentures, hearing aids, and glasses.
  • Pack up all patient belongings at discharge.

Requirements:

  • Completion of CNA coursework within the past 24 months, or successful completion of Carle's HCT In Training Program, or work experience as a CNA within the past 24 months, or successful completion of at least one semester of nursing clinical experiences, or documented relevant medical military training.
  • Use of usual and customary equipment used to perform essential functions of the position.
  • Basic computer knowledge using word processing, spreadsheet, email, and web browser.
