Job Overview

As a Clinical Manager Home Health, you will play a pivotal role in directing, coordinating, and evaluating the delivery of home health services. You will ensure compliance with agency policy, state and federal home health regulations, and drive the management of cost-effective, efficient care to meet the individual needs of patients. Your expertise will serve as a role model and mentor for the implementation of our organization's Mission and Core Value statements.

Responsibilities

  • Direct clinical supervision, performance evaluations, and ensure competency requirements are met for all staff.
  • Collaborate with HR in aspects of personnel management, including interviewing, hiring, and disciplinary action.
  • Promote efficient utilization of resources to achieve desired clinical outcomes and within episode reimbursement.
  • Provide effective guidance to professional staff regarding home health service utilization, clinical interventions, and patient care requirements.
  • Conduct individual caseload reviews, team meetings, and case conferences as needed to meet the needs of challenging clients.
  • Take a proactive approach in identifying high utilization and problem-prone cases, and take appropriate action.
  • Ensure appropriate levels of clinical coverage based on census, multidisciplinary skill mix, geographic distribution, and patient care requirements.
  • Participate in administrative on-call managerial coverage for weekends and holidays, and arrange for managerial coverage when off.
  • Research and follow through with customer and employee complaints in a timely fashion.
  • Enhance professional growth through attendance at in-services, conferences, and professional organizations.
  • Evaluate staff understanding of documentation and compliance with regulations, and review payroll documentation within agency guidelines.
