Job Title: Home Health Registered Nurse

At Saint Luke's Health System, we are seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

About the Role:

This is a part-time position, working Monday-Friday with flexibility during daytime hours. On-call as assigned. You will be responsible for coordinating patient care, consulting with other healthcare professionals, and facilitating patient education and support.

Responsibilities:
  • Utilize the nursing process to provide patient-centered care
  • Collaborate with the medical staff and other disciplines to enhance patient outcomes
  • Adhere to all nursing policies and standards, striving to maintain and improve quality outcomes
  • Work autonomously with a supportive team, communicating effectively to ensure seamless patient care
