VNS Health is seeking a skilled Physical Therapist to join our team in Thornwood, New York.
Job SummaryWe are looking for a compassionate and experienced Physical Therapist to provide high-quality rehabilitation services to our patients in their homes and communities. As a Physical Therapist with VNS Health, you will have the opportunity to work independently as part of an interdisciplinary care team, conducting comprehensive assessments and developing personalized treatment plans to help patients achieve their goals.
Responsibilities- Conduct thorough assessments of patients to determine their needs and develop effective treatment plans.
- Deliver personalized rehabilitation therapy to patients in their homes or care facilities, using evidence-based practices and techniques.
- Evaluate patient progress and adjust treatment plans as needed to ensure optimal outcomes.
- Collaborate with other healthcare professionals to provide comprehensive care and support to patients and their families.
- Minimum of one year of experience as a Physical Therapist or successful completion of a VNS Health Rehab Internship program.
- Licensure to practice as a Physical Therapist in New York State.
- Valid driver's license required for all areas outside of Manhattan.
- Competitive pay rates, with a hiring range of $70-$75 per visit for all areas outside of the Bronx and $90-$95 per visit for the Bronx.
