About the Role

VNS Health is seeking a skilled Physical Therapist to join our team in Thornwood, New York. As a Physical Therapist, you will play a vital role in helping New Yorkers recover from surgery, illness, or accident and regain their independence in the comfort of their own homes and communities.

Key Responsibilities
  • Conduct comprehensive assessments of patients to determine their needs and develop personalized rehabilitation therapy plans.
  • Deliver high-quality, one-on-one therapy sessions to patients in their homes or care facilities.
  • Evaluate patient progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Collaborate with interdisciplinary care teams to provide comprehensive care and support to patients.
Requirements
  • Minimum of one year of physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
  • Licensure to practice as a Physical Therapist in New York State.
  • Valid driver's license required for all areas outside the borough of Manhattan.
