The role of a Clinical Pharmacist Specialist is integral to a dedicated patient care team focused on ensuring safe and effective medication management for unique patient populations.


These specialists are essential in delivering direct, patient-centered care, collaborating with multidisciplinary teams, and incorporating clinical pharmacy services into the overall care framework.

Key responsibilities encompass evaluating patient requirements, modifying drug therapies, and mitigating medication-related adverse events. Furthermore, they engage in research activities, support quality improvement initiatives, and offer education and training to peers.



Key Responsibilities:

  • Oversee patient service lines, clinical domains, and therapeutic initiatives
  • Manage pharmaceutical care services and medication utilization systems
  • Develop and execute stewardship activities and monitoring programs
  • Assess and monitor pharmacy programs for operational, quality, and financial effectiveness


Qualifications:

  • Graduate from an ACPE accredited College of Pharmacy
  • B.S. in Pharmacy or Doctor of Pharmacy (PharmD) degree
  • Active licensure as a Pharmacist in the practicing state
  • Completion of an ASHP accredited PGY2 Pharmacy Residency OR PGY1 Pharmacy Residency plus three years of clinical pharmacy experience in a specialized field OR six years of clinical pharmacy experience in a specialized area
  • In-depth knowledge and expertise in advanced pharmacy practice, comprehensive medication management, and evidence-based medicine relevant to clinical responsibilities

Certifications and Clearances:

  • Registered pharmacist with necessary licensure/eligibility
  • Pharmacist Act 34 with renewal
