Job Summary

UPMC is seeking a highly skilled Clinical Pharmacy Specialist - Oncology to join our team. As a key member of our patient care team, you will be responsible for providing safe and effective medication management for specialized patient populations.

Key Responsibilities
  • Lead assigned patient service lines, clinical areas, and therapeutic programs to ensure seamless delivery of pharmaceutical care services.
  • Facilitate pharmaceutical care services, direct patient care programs, and medication utilization systems to align with patient care needs, evidence-based best practices, and regulatory standards.
  • Design and implement stewardship activities and restriction/surveillance programs to optimize medication use and minimize adverse events.
  • Track and evaluate assigned pharmacy programs for operational, quality, and financial efficiency, and benchmark against local and national best practices.
  • Identify practice-related issues and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education to advance practice.
  • Develop and oversee policies and procedures for drug purchasing, drug usage, drug distribution, and drug control to ensure pharmacy is an integral part of the healthcare delivery system.
  • Deliver direct patient care and clinical practice, including decentralized and service-based programs, to provide patient-centered care.
  • Maintain proficiency in decentralized pharmacy services and clinical pharmacy programs to ensure optimal patient outcomes.
  • Work as an active member of a multidisciplinary team and collaborate with healthcare providers within decentralized patient care areas to provide patient-centered care.
  • Identify high-risk patients and implement interventions to improve quality and safety.
  • Make appropriate evidence-based, patient-centered medication recommendations to optimize patient outcomes.
  • Participate in the management of medical emergencies and provide discharge medication review, reconciliation, and counseling as appropriate.
  • Provide pharmaceutical services throughout the medical center and maintain proficiency in hospital computer systems and medication ordering systems.
  • Provide accurate, safe, timely, and appropriate medication therapy based on patient age and needs.
  • Complete critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, subtherapeutic dosage, overdose, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.
  • Communicate effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and assure continuity of pharmaceutical care between shifts and among staff.
  • Actively participate in stewardship activities and restricted medication programs to optimize medication use and minimize adverse events.
  • Participate in pharmacy operations and medication dispensing to ensure seamless delivery of pharmaceutical care services.
  • Maintain proficiency in and actively engage in operational programs, central dispensing pharmacies, satellite dispensing pharmacies, and specialty pharmacy areas as appropriate for job assignment.
  • Facilitate specialty medication procurement, ordering, and dispensing procedures, including but not limited to chemotherapy, parenteral nutrition, controlled substances, etc. as appropriate for assignment.
  • Oversee pharmacy operations and technician practice/activities to ensure optimal patient outcomes.
  • Facilitate experiential education and practice advancement to develop student, resident, and staff training experiences/competencies.
  • Maintain proficiency in preceptor roles and actively emulate education/mentorship skills to develop student, resident, and staff training experiences/competencies.
  • Develop student, resident, and staff training experiences/competencies and create new relationships for teaching and training opportunities.
  • Identify, design, and implement improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs.
Requirements
  • Graduate of an ACPE accredited College of Pharmacy.
  • B.S. degree in Pharmacy or Doctor of Pharmacy (PharmD) degree; PharmD preferred.
  • Required: Current licensure as a Pharmacist in practicing state.
  • Completion of ASHP accredited PGY-2 Pharmacy Residency OR PGY-1 Pharmacy Residency plus 3 years clinical pharmacy experience in a specialized area OR 6 years clinical pharmacy experience in a specialized area.
  • Specialized knowledge and expertise in advanced pharmacy practice and service, comprehensive medication management, and evidence-based medicine in specialized area appropriate for clinical assignments.
  • BPS Board Certification preferred.
