Our Department

Our Internal Medicine department provides care for adults, with a focus on preventive care and the treatment of complex adult diseases. As a Population Health Coordinator, you will work closely with our team of healthcare professionals to ensure seamless patient care.

Responsibilities
  • Document patient interactions in a concise manner, compliant with documentation requirements
  • Conduct in-bound and out-bound calls, including patient scheduling, screenings, and reminder calls
  • Assist with scheduling and coordinating patient transportation and follow-up physician appointments
  • Utilize reports to track patient referrals and scheduling no-shows, in collaboration with practice staff
  • Provide clerical and administrative support to clinical staff and managers for special studies, projects, and reports
  • Provide excellent customer service, serving as a resource to internal and external customers
  • Maintain knowledge of all health plan benefits and payor requirements
  • Call patients as designated by PCP or clinical staff
  • Verify patients have been seen and follow up on no-show patients
  • Assist patients with barriers
  • Ensure appropriate clinical information is communicated to clinical staff when there is variance from plan of care
  • Coordinate other medical office assistants in outreach and report validation needs
Education and Licenses

Valid NYS driver's license, current Medical Assistant certification or registration preferred, high school graduate or GED equivalent preferred, and more than one year of related experience in a medical setting.

Physical Requirements

Medium work, exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing, or squatting.
