We are seeking a highly skilled and compassionate Hospice Clinical Liaison to join our team at FC Compassus LLC. As a key member of our organization, you will play a vital role in promoting our hospice philosophy and values, while providing exceptional care to our patients and their families.
Key Responsibilities- Develop and maintain strong relationships with referral sources, including hospitals, facilities, and community organizations.
- Assess patient needs and disease status, conferring with attending physicians and hospice physicians to establish eligibility for our services.
- Facilitate the admission process, ensuring seamless transitions from hospital to home or long-term care settings.
- Provide education and support to patients, families, and healthcare professionals on our services and benefits.
- Collaborate with our team to identify and address symptom management needs, ensuring optimal patient outcomes.
- Perform other duties as assigned, contributing to the growth and development of our organization.
- RN License required; Bachelor's degree in Nursing highly preferred.
- Two to three years of related experience, with hospital and/or long-term care clinical experience highly preferred.
- Strong communication and interpersonal skills, with the ability to articulate our hospice philosophy and values.
