Key Responsibilities:- Provide direct patient care, including assessment, treatment, and education, in accordance with established standards and practices.
- Collaborate with physicians and staff to coordinate patient care, ensure seamless transitions, and promote effective communication.
- Function as a primary liaison between patients, staff, and physicians, ensuring that patient needs are met and concerns are addressed.
- Participate in nurse triage, evaluating patient situations, and determining the urgency of care, directing patients to the appropriate site of care, and ensuring accurate documentation of consultations and treatments.
- Develop and implement individualized care plans, incorporating patient preferences, values, and goals, and ensuring that care is patient-centered and evidence-based.
- Stay up-to-date with the latest clinical guidelines, research, and best practices, applying this knowledge to improve patient outcomes and enhance the quality of care.
- Contribute to quality improvement initiatives, identifying areas for improvement, and developing strategies to address these gaps, ensuring that care is safe, effective, and efficient.
- RN degree from an accredited school of nursing, with a minimum of one year of professional nursing experience, preferably in a clinic setting.
- Current state registered nurse license, CPR certification, and ACLS certification (if required for the role).
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
- Ability to work in a fast-paced environment, prioritizing tasks, and managing multiple responsibilities with minimal supervision.
