We are seeking a skilled Physical Therapist to join our team at LHC Group, providing care and support to patients in their homes.
**About the Role:**
As a Physical Therapist with LHC Group, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, you will work with patients to develop personalized treatment plans aimed at improving and enhancing their well-being. You will also evaluate patient progress and report any changes to the supervising nurse or physician.
**Responsibilities:**
1. Evaluate patient's functional status and physical therapy needs.
2. Assist the physician in developing the therapy plan of care.
3. Observe and report patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
4. Treat patients to relieve pain, develop, and restore function, and/or maintain maximum functioning using acceptable standards of practice.
5. Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
