Key Responsibilities- Conduct thorough assessments of patients and families to determine their home care needs and develop personalized care plans.
- Provide direct patient care, including administering medications and treatments as prescribed by physicians, and educating patients and families on health and wellness.
- Collaborate with community health professionals to coordinate care and ensure seamless transitions between healthcare settings.
- Supervise and evaluate the care provided by home health aides and other support staff.
- Communicate effectively with patients, families, and healthcare teams to ensure that patient needs are met and care plans are implemented.
- Participate in on-call duties and respond to patient needs outside of regular business hours.
- Graduate of an accredited school of nursing with a current license to practice in the state.
- One to two years of recent acute care experience in an institutional setting, with a preference for home health care experience.
- Current CPR certification and a valid driver's license with a reliable vehicle.
- Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
