Job Description

The Pediatric Health Specialist will be responsible for coordinating the operational and administrative functions of the Mobile Health Program through Healthy Communities.

This role will work closely with Healthy Communities and other divisions and programs within the hospital to ensure the Mobile Health Program delivers quality clinical services in the communities of Chicago.

In addition, this individual will support other programming efforts in which the mobile unit participates.

Key Responsibilities

  1. Assist providers in the delivery of nursing care on the mobile unit by performing assessments, obtaining vital signs, administering medication, documenting patient data, and all other duties as assigned.
  2. Perform administrative operations, including receiving, reviewing, and processing patient registration packets received from schools and community partners.
  3. Serve as a Vaccines for Children (VFC) coordinator, verifying patient eligibility, maintaining vaccine ordering and accountability, and facilitating IDPH site visits and audit processes.
  4. Collaborate with healthcare teams to identify healthcare and clinical needs of patients and develop strategies to address these needs.

Required Skills and Qualifications

  • Bachelor's degree in Nursing (BSN) from an accredited school and current licensure as a registered professional nurse in Illinois, active and in good standing.
  • Minimum of 2 years of nursing experience, with at least 1 year in ambulatory care.
  • Experience in vaccine administration and knowledge of Epic EHR system preferred.
  • Fluency in Spanish (verbal and written) desired.
  • Ability to work independently, problem-solve, and support teamwork in a diverse setting.
