Role Summary:
The RN Case Manager is responsible for conducting comprehensive admission evaluations and identifying patients who may not meet specific criteria, ensuring they receive the most suitable level of care. This role involves active participation in daily care coordination meetings with the healthcare team to facilitate effective discharge planning and optimal patient outcomes.
Core Duties Include:
- Conducting timely assessments to determine appropriate hospital care levels based on clinical indicators.
- Collaborating with the healthcare team to maintain a continuous evaluation of patient care quality and appropriateness.
- Developing and updating individualized care plans based on patient needs and team discussions, ensuring documentation in the electronic medical record (EMR).
- Coordinating discharge planning with relevant internal and external providers while safeguarding patient confidentiality.
- Documenting care processes professionally and timely, adhering to quality review standards.
- Advocating for patients and their families with respect and confidentiality.
- Serving as a resource for the healthcare team regarding case management processes and patient care.
- Referring complex cases to appropriate management when necessary.
- Ensuring compliance with Medicare documentation requirements where applicable.
- Addressing consults related to case management effectively.
Additional Responsibilities:
- Collaborating with social workers to identify and address social and financial barriers affecting patient care.
- Participating in departmental meetings and committees to enhance service delivery.
- Engaging in continuous professional development through educational opportunities and literature review.
- Providing guidance to healthcare providers on financial and clinical management issues.
- Maintaining adherence to all organizational policies and HIPAA regulations.
- Performing other related duties as assigned.
Required:
- Current licensure or eligibility for licensure in Kentucky.
- Basic Life Support (BLS) certification from an accredited organization.
- Bachelor's degree in nursing or a related field preferred; consideration for RNs with an Associate's Degree pursuing a BSN.
- A minimum of 2-3 years of nursing experience, with a focus on utilization management or case management.
Preferred:
- Certification in a nursing specialty or as a Certified Case Manager (CCM).
