Key Responsibilities:
- Provide direct patient care, including assessment, diagnosis, outcomes/planning, implementation, and evaluation
- Collaborate with residents, families, and healthcare teams to develop and implement individualized care plans
- Conduct resident assessments, develop care plans, and implement interventions to promote resident health and well-being
- Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination
- Participate in quality improvement initiatives to enhance resident care and satisfaction
Requirements:
- Graduate from a nationally accredited nursing program
- Currently holds an unencumbered RN license with the State Board of Nursing
- Obtains and maintains required department-specific competencies and certifications
