About the Role

As a Home Health LPN, you will provide high-quality patient care in the patient's home, working closely with a team of healthcare professionals to deliver exceptional results. Your responsibilities will include taking patient vitals, conducting assessments, documenting progress, and reporting changes to the RN supervisor.

Requirements
  • Graduate of an accredited practical nursing program
  • Active LPN license in the state of Colorado
  • Minimum of 1200 hours of LPN experience, ideally in home healthcare but not mandatory
  • BLS/CPR certification (demonstration course required)
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills, and compassion
  • Pass a background check and drug test (we are required to test for THC)
