Home Health Licensed Practical Nurse

At Interim HealthCare, we're committed to delivering exceptional patient care and creating a supportive work environment for our nurses. As a Home Health Licensed Practical Nurse, you'll play a vital role in providing high-quality care to patients in the comfort of their own homes.

Key Responsibilities:
  • Provide comprehensive home-based care to patients with various medical conditions, under the supervision of an RN
  • Assist with daily activities such as bathing, dressing, and grooming, as well as provide nutritional support and hygiene care
  • Collaborate with a multidisciplinary team to develop and implement patient care plans, ensuring seamless transitions and optimal outcomes
  • Take vital signs, perform assessments, and document patient progress, reporting any changes to the RN supervisor
  • Teach patients and their families self-care techniques and provide education on medication management and home safety
Requirements:
  • Graduate of an accredited practical nursing program and active LPN license in Colorado
  • Minimum of six (6) months of LPN experience, preferably in home healthcare
  • Wound and/or Med Surg experience is a plus
  • CPR Certification (demonstration course required)
  • Knowledge of state and federal home health regulations
