POSITION SUMMARY:

The Social Work Case Manager plays a vital role in collaborating with the healthcare team to ensure the seamless delivery of care for patients. This position focuses on minimizing delays and denials in patient services.

KEY RESPONSIBILITIES:

  • Actively coordinate with community organizations to address issues such as abuse, neglect, substance misuse, and mental health challenges.
  • Facilitate connections to financial resources and support services for patients and their families.
  • Lead initiatives to improve patient care outcomes through effective communication and teamwork.

QUALIFICATIONS:

  • A Master’s degree in Social Work is mandatory.
  • Experience in Social Work ranging from 2 to 4 years is essential.
  • Prior experience in Medical Social Work is highly regarded.
  • Certification as a Licensed Clinical Social Worker (LCSW-C) is required.
