As a key member of our interdisciplinary team, you will play a vital role in providing high-quality nursing care to our residents across all three communities.
Responsibilities- Clinical Care: Conduct comprehensive assessments of patients' health needs, develop personalized care plans, administer medications and treatments, and monitor vital signs and symptoms.
- Supportive Services: Assist patients with activities of daily living, offer emotional support and companionship, and create a safe and comfortable environment.
- Documentation and Communication: Maintain accurate and up-to-date medical records, communicate effectively with patients, families, and colleagues, and participate in interdisciplinary meetings.
- Educational Support: Educate patients and families on health management practices, provide guidance and training to caregivers and staff, and contribute to quality improvement initiatives.
- Registered Nurse (RN) license in the state
- Experience in geriatric or long-term care preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work effectively within a team and independently
