Job Summary:

The Aftercare Clinician plays a vital role in our team, providing essential clinical services to youth and families transitioning from our residential program. This includes delivering individual, group, and family therapy sessions, as well as maintaining effective communication with various stakeholders.

Key Responsibilities:
  • Provide therapeutic services to youth and families in their home and community setting, fostering a supportive environment for growth and development.
  • Interact professionally with youth, families, and staff members to achieve positive outcomes and build strong relationships.
  • Maintain positive working relationships with families and county representatives through regular collaboration and open communication.
  • Implement agency-approved policies, protocols, and procedures to ensure the highest standard of care is consistently delivered.
  • Accurately maintain record-keeping and reports, ensuring seamless communication within the organization.
  • Demonstrate exceptional communication and time management skills to effectively manage multiple priorities and responsibilities.
Requirements:
  • A Master's Degree or equivalent in the mental health field (LMSW, LMFT, LMHC, etc.).
  • Experience working in a mental health environment, with a proven track record of success.
  • In-depth knowledge of community resources and support systems, enabling effective referrals and collaborations.
  • Ability to work independently and collaboratively as part of a multidisciplinary team, promoting a culture of cooperation and shared goals.
  • A valid Driver's License and proof of automobile insurance are required for this position.
