Catholic Social Services of the Diocese of Scranton is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive case management services to individuals, including needs assessments, case plan development, and documentation.
Key Responsibilities- Administrative: Adhere to established regulations, policies, and procedures of the agency and program. Attend trainings to enhance skills and job performance as directed by the program supervisor.
- Case Management/Direct Service: Provide weekly/bi-weekly case management/supportive services to residents, including developing and implementing an individual service plan. Provide supervision of residents while at the shelter and during outside events. Prepare and maintain records as required by specific program and state regulations.
- Supervision: Report to the Program Supervisor for direction, monitoring, and evaluation of job performance.
- Bachelor's degree in a Human Services Discipline
- Valid PA driver's license
- Ability to navigate stairs and lift at least 50lbs
