We are seeking a highly skilled and dedicated MDS Coordinator to join our leadership team at St. Clare Commons. As a key member of our team, you will play a vital role in ensuring the accuracy of our resident assessments and care plan optimization.
Key Responsibilities:- Resident Assessment and Care Planning: Conduct and coordinate the development and completion of resident assessments (MDS) by current rules, regulations, and guidelines.
- Timely Completion: Ensure accurate and timely completion of the MDS process.
- Interdisciplinary Collaboration: Collaborate with the interdisciplinary team to develop individualized resident care plans.
- Progress Monitoring: Monitor and document resident progress and responses to care plans.
- Quality Assurance: Participate in the Quality Assurance and Performance Improvement (QAPI) process.
- Communication: Communicate with residents, families, and other healthcare providers to coordinate optimal care delivery.
- Regulatory Compliance: Stay updated on MDS regulations and guidelines to ensure compliance.
- Competitive Pay: Up to $84,000 annually.

- Current RN License: Current RN (Registered Nurse) license.
- RAC-CT Certification: RAC-CT (Resident Assessment Coordinator-Certified) certification is required.
- MDS Knowledge: Strong understanding of MDS 3.0, RAI process, and care planning.
- Excellent Skills: Excellent organizational, communication, and interpersonal skills.
- Teamwork: Ability to work independently and collaboratively within a team.
- Commitment to Quality Care: Commitment to providing high-quality care to residents.
This is a full-time, on-site position at St. Clare Commons.
