We are seeking a highly skilled and dedicated MDS Coordinator to join our team at St. Clare Commons. As a key member of our leadership team, you will play a vital role in ensuring the accuracy and timeliness of our resident assessments and care plans.

Key Responsibilities:
  • Develop and implement effective resident assessment strategies to ensure compliance with current regulations and guidelines.
  • Collaborate with our interdisciplinary team to create individualized care plans that meet the unique needs of our residents.
  • Monitor and document resident progress, ensuring that care plans are optimized for maximum effectiveness.
  • Participate in our Quality Assurance and Performance Improvement (QAPI) process to drive continuous quality improvement.
  • Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
  • Stay up-to-date on the latest MDS regulations and guidelines to ensure our facility remains compliant.

Requirements:
  • A current RN (Registered Nurse) license.
  • RAC-CT (Resident Assessment Coordinator-Certified) certification is required.
  • A strong understanding of MDS 3.0, RAI process, and care planning.
  • Excellent organizational, communication, and interpersonal skills.
  • The ability to work independently and collaboratively within a team.
  • A commitment to providing high-quality care to our residents.
