Key Responsibilities- Provide comprehensive pharmaceutical care to patients, including medication therapy management and education.
- Collaborate with the healthcare team to optimize patient outcomes and quality of life.
- Develop and implement individualized medication plans to meet the unique needs of each patient.
- Monitor and evaluate patient responses to medications, making adjustments as needed.
- Stay up-to-date with the latest advancements in pharmacy practice and apply this knowledge to improve patient care.
- PharmD in Pharmacy or Bachelor's Degree in Pharmacy.
- Oregon Pharmacist License upon hire.
- Acute care hospital pharmacy experience, 1 year or equivalent experience.
- Completion of ASHP-accredited PGY1 residency.
