About the Role

Sunrise Senior Living is seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for providing exceptional care and services to our residents and their families in our assisted living and reminiscence neighborhoods.

Key Responsibilities
  • Build Meaningful Relationships: Develop strong relationships with a specified number of seniors and their families, providing assistance with activities of daily living, attending to individual care needs, and getting to know their unique preferences and personalities.
  • Notify Management of Changes: Inform management of any changes in a resident's condition and recommend adjustments in the level of care and service as needed.
  • Assist with Life Skills: Assist residents in life skills and other life-enriching activities as indicated on their individual profile, blending a variety of multi-sensory experiences into their daily routine.
  • Participate in ISP Development: Contribute to the development of Individualized Service Plans (ISPs) and monthly updates, ensuring that each resident's needs are met and exceeded.
  • Responsible for Resident Care: Be responsible for a designated group of residents during the shift, knowing their whereabouts and physically checking on them throughout the shift.
  • Partner with Community Team: Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies, adhering to safety rules and regulations.
  • Respond to Dining Needs: Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
  • Encourage Resident Engagement: Invite, encourage, and assist residents in life skills and other life-enriching activities as indicated on their ISP and demographic profile.
Requirements
  • Dedication to Serving Seniors: Demonstrate a passion for serving seniors with excellent customer service skills.
  • Positive Attitude and Flexibility: Possess a positive attitude and the flexibility to perform various duties in service to residents and the ability to work in a team environment.
  • Education and Certifications: High School diploma/GED accepted and may be required per state regulations. In states where appropriate, must maintain certifications.
  • Previous Experience: Previous experience working with seniors preferred.
  • Decision-Making and Problem-Solving: Ability to make choices, decisions, and act in the resident's best interest.
  • Effective Communication: Possess written and verbal skills for effective communication and a level of understanding.
  • Organizational and Time Management: Competent in organizational and time management skills.
  • Good Judgment and Problem-Solving: Demonstrate good judgment, problem-solving, and decision-making skills.
