Social Work Manager

St. Ann's Community is seeking a compassionate and experienced Social Work Manager to lead the Social Work Department in our Portland campus. As a key member of our team, you will be responsible for managing the day-to-day functions of the department, ensuring that our residents and patients receive high-quality social work services.



Key Responsibilities:

  • Coordinate the social work program for St. Ann's Home, Wegman Care Center, and Leo Center for Caring
  • Plan, develop, and administer all Social Work programs and services
  • Demonstrate knowledge of general social work practices and adherence to state and federal regulations
  • Maintain a quality improvement program, including policy review, auditing, and documentation
  • Ensure that problems emergent in Social Work practice are identified and resolved in a timely fashion
  • Establish and maintain relationships with residents, patients, and significant others, providing counseling, support groups, and referrals as needed
  • Continually assess residents' and patients' psychosocial strengths and needs, providing counseling, support groups, and referrals to meet identified needs
  • Act as a liaison between the family and St. Ann's staff, providing supportive services and interpreting policies and procedures
  • Provide social work input to address psychosocial aspects of all resident and patient care needs, including behavioral issues, psychotropic medication use, and physical/cognitive decline
  • Coordinate Social Work activities with other major departments, including Administration, Admissions, Nursing, Medical, Dietary, Recreation Therapy, Facility Services, Finance, and Pastoral Care
  • Actively maintain quality working relationships with other departments and external agencies
  • Maintain liaison relationships with community agencies essential in the execution of the Social Work Mission in St. Ann's
  • Serve as a liaison to the Hospice Agencies contracted with St. Ann's Community
  • Serve as a liaison between United Health Care Optum personnel and the Social Work department
  • Serve as a liaison to the Sr. Psychology Services Program
  • In conjunction with the Life Enrichment Coordinator, actively pursue ways for St. Ann's Home to move forward in the culture change journey
  • Actively seek out and support education regarding person-centered care for self, staff, and others within the organization
  • Serve as a role model and assist the organization in ensuring that the needs, routines, wants, and desires of the individual are supported and met
  • Demonstrate knowledge and adherence to state and federal regulations in administering the St. Ann's, WTCC, and LCC social work policies and procedures
  • Review, update, and/or develop new social work policies and procedures annually
  • Ensure that policies and procedures are communicated to staff effectively
  • Conduct bi-weekly staff meetings and maintain minutes of meetings
  • Complete monthly rounding with all direct reports and submit to supervisor
  • Assure quality working and liaison relationships with internal and external parties
  • Create and maintain a positive working environment
  • Coordinate St. Ann's Home Resident Council and Family Council
  • Coordinate St. Ann's Home Quarterly Newsletter with the Life Enrichment Coordinator
  • Maintain timely documentation of events and all required paperwork, including MDS, CAAS, and ICCP
  • Coordinate resident/patient group meetings, including Resident Council and family/resident/patient support and informational groups
  • Coordinate transportation needs for residents/patients on an as-needed basis
  • Assist residents/family and significant others in making application to MCDSS for Medicaid and provide interpretation and assistance with health-related forms
  • Provide supervision of social work students from area colleges
  • Facilitate the application of Resident's/Patient's Rights in daily care issues
  • Provide rotating weekend social work coverage
  • Perform tasks as assigned by the Manager


Candidate Requirements:

  • Master's Degree in Social Work (M.S.W.) or Gerontology preferred, Bachelor's degree in Social Work (B.S.W), Gerontology; four to ten years related experience and/or training; or equivalent combination of education and experience
  • Three years' minimum experience in Geriatric Social Work, including supervisory experience
