Key Responsibilities
  • Conduct thorough assessments of members' physical and behavioral health needs, including social determinants of health.

  • Collaborate with healthcare providers, caregivers, and community resources to coordinate services and ensure seamless transitions in care.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to achieve optimal health outcomes.
Requirements
  • A minimum of 3 years of experience in a similar role or a related field, such as nursing, social work, or healthcare administration.
  • Current RN license in applicable state(s) required.
  • Strong understanding of healthcare systems, policies, and regulations.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
Preferred Qualifications
  • MA/MS in Health/Nursing preferred.
  • Experience working with populations with complex medical and social needs.
Compensation

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience and qualifications. This estimate is based on industry standards and location-specific costs of living.
