Job Overview

The Licensed Vocational Nurse (LVN) plays a vital role in providing high-quality care to patients at Healthcare For the Homeless.

This position is ideal for an individual who can work effectively in a team environment, manage time efficiently, and prioritize tasks while maintaining professionalism in appearance and actions.

As a key member of our healthcare team, the LVN will provide clinical expertise, patient education, and support staff assistance to ensure seamless delivery of care.

Key Responsibilities:

  • Serves as a resource for teams of Medical Assistants (MA) and Providers, providing patient education on self-care, medications, nebulizers, inhalers, diabetic education, wound care, and self-injections.
  • Oversees the intake of patient telephone calls, provides advice on treatment of minor acute problems, refers questions to the PCP or APC as needed, and assists support staff, MA's and supervisors with message taking and emergency protocols.
  • Participates in patient follow-up activities such as reporting diagnostic test results, arranging prescription refills, expediting referrals, performing prior authorizations, and preparing information for prescription renewals at the request of the PCP/APC.
  • Tries walk-in/same-day patients and manages the intake and monitoring of patients with acute medical conditions, monitors patients in the absence of the PCP/APC, provides necessary nursing intervention when appropriate, and performs procedures as determined by the clinical needs of the agency, including but not limited to: IM and SC injections, nebulizer and inhaler treatments, EKG's, administering and reading PPD's, suture removals, ear lavage, wound care/dressing changes, and other procedures within the scope of professional LVN nursing practice.

Estimated Salary Range: $41,601 - $57,410 per year.
