About the Job

We are seeking a dedicated and passionate Licensed Practical Nurse (LPN) to join our team at BrightSpring Health Services. As a Home Health LPN, you will have the opportunity to provide essential one-on-one care to patients in their homes, while also growing your nursing skillset and enjoying the benefits of flexibility and autonomy.

Key Responsibilities
  • Provide safe patient care and deliver individualized nursing care plans to ensure patients have an optimal care experience.
  • Work as part of an interdisciplinary team to support patients and their families.
  • Monitor patients' vital statistics and report abnormal findings to the Case Manager and Physician.
  • Administer medications, feedings, oxygen, and ostomy care as needed, and assess the results.
  • Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
  • Ensure infection control policies are maintained at all times and report possible communicable diseases or infections.
  • Accurately document nursing actions, communication with family, case manager, and physicians, and document progress and outcomes for established goals.
  • Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
  • Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
Requirements
  • LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
  • Valid driver's license, acceptable driving record, and proof of car insurance.
  • CPR professional certification.
  • Experience in administering medications.
  • 1+ years of nursing experience preferred.
  • Home health care experience preferred.
  • Able to lift and carry items weighing up to 20 pounds.
  • Able to bend, stoop, push, pull, sit, and walk for periods of time.
  • Must be able to communicate both verbally and in writing.
