Nurse Care Specialist

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at SpringHill Home Health and Hospice. As a Home Health LPN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.
Key Responsibilities:
- Provide individualized nursing care to patients in their homes, following their personalized care plans.
- Monitor patients' vital signs and report any abnormalities to the Case Manager and Physician.
- Administer medications, feedings, oxygen, and other treatments as needed, and document the results.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordinate supplies and equipment as needed.
- Ensure infection control policies are maintained and report any incidents or concerns to the Case Manager.
Requirements:
- LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
- Valid driver's license, acceptable driving record, and proof of car insurance.
- CPR professional certification.
- Experience in administering medications and providing home health care preferred.
